NOTE:

1. This application form is non – refundable fee of Tsh 30,000/= each.  Should be submitted day of interview.

2. The application form does not constitute a promise that your child will be accepted.

3. Please come with copy of your Birth Certificate, Previous Report and passport size image photograph the day of interview

4. For Interview you will be Notified by call or SMS.  Interview will take place at the school premises starting from 8.30 am.
